GILES HIGH SCHOOL School Dance Guest Registration  

A Giles High School student is requesting to bring a Guest who is NOT a current Giles High School student to a school sponsored dance or event. This form must be completed BEFORE purchasing a ticket to the dance and must also be turned in to Giles High School at least two days prior to the Dance or event. The Guest must have this form completed ALONG WITH A PHOTOCOPY OF HIS/HER SCHOOL ID, STATE ID. OR DRIVER'S LICENSE (OR OTHER ID). In addition, the guest must present his/her ID at the door the evening of the dance or he/she WILL NOT BE ALLOWED TO ENTER. Students who have failed to complete required courses at GHS or are considered a drop-out may not attend GHS dances. Students may not attend GHS dances if they are more than one year removed from High School (exception to prom). Students who are 20 years old or younger may attend Prom. No one over 20 years old will be admitted.

As a GHS student I understand that I am responsible for my guest. I further understand that failure to comply with GHS policies may result in dismissal from the event as well as disciplinary action against me. As a Giles student I am also responsible for informing my guest of these rules.
